Before your appointment

A short questionnaire arrives by email when you book. Completing it is the single most useful thing you can do beforehand.

  • Complete the pre-appointment questionnaire so your clinician arrives already read in
  • Write down when the problem started and what makes it better or worse
  • List everything you take, including supplements and anything bought online
  • Gather letters, results or imaging from clinicians outside the practice
  • Tell us at booking if you would prefer a clinician of a particular gender
  • Let us know if you need an interpreter; we arrange it rather than asking you to

What to bring

Very little is required. These few things make the appointment more useful.

  • Photo identification for a first visit
  • Your insurer details and pre-authorisation code, if you are using insurance
  • Your medication list, or the boxes themselves if that is easier
  • Photographs of anything visible that comes and goes
  • A list of the two or three questions you most want answered

Arriving

Arrive five minutes early. You will be checked in at the desk and shown to the floor you need.

  • Check in at the ground-floor desk; there is no kiosk and no queue system
  • Waiting areas are separate from the consulting floors at every site
  • A quiet waiting area is available at every location if you ask when you book
  • You may bring somebody with you into the appointment
  • If you are running late, call ahead — we can usually still see you

Accessibility

Every site is step-free from the street. Specific provision differs by building and is listed on each location page.

  • Step-free entrances and lifts to every consulting floor
  • Accessible WCs at all four sites; an accessible shower at Ashbourne Mews
  • Hearing loops at reception desks and in selected consulting rooms
  • Adjustable-height examination couches and an imaging hoist at Wharfside
  • Assistance dogs are welcome throughout every building
  • Tell us what would help when you book, and it will be arranged before you arrive

Prescriptions and repeats

Prescriptions are issued electronically and can be collected from the pharmacy of your choice.

  • New prescriptions are issued during or immediately after the appointment
  • Repeats are requested through the practice and reviewed by your named clinician
  • Allow two working days for a repeat request, longer over a weekend
  • Some medication requires a review appointment before it can be reissued
  • Travel prescriptions are handled through the travel health appointment

Results

Results are explained by a clinician. Nothing is released as a number on its own.

  • A results conversation is booked at the same time as any test that needs one
  • Most tests booked before midday at Wharfside are reported the same working day
  • You will be contacted whether the result is expected or unexpected
  • Copies of your results are provided in writing after the conversation
  • If you feel unwell while waiting, contact the practice rather than waiting for the appointment

Setting up a virtual appointment

A link arrives by email and text. There is no app to install and no account to create.

  • Open the link a few minutes early to settle camera and microphone permissions
  • Use headphones if you have them; they improve the call more than the camera does
  • Sit with light in front of you rather than behind you
  • A telephone appointment is always available instead of video
  • If the connection fails, your clinician will call the number on your record

Safety and consent

What you can expect from us, every time

These apply at every site, to every appointment, whoever you see.

Consent before examination

Every examination is explained before it starts, a chaperone is offered every time, and you can stop at any point without giving a reason.

A named clinician

Each patient has a named clinician who is responsible for their care and for replying to them.

Escalation, in writing

If something needs urgent or emergency care, we say so plainly, arrange it where we can, and write down what to do if it changes overnight.

Learning from what goes wrong

Complaints and near misses are reviewed monthly by the clinical team, and the changes that follow are recorded.

Your record

Who can see what

  • Your record is visible to the clinicians involved in your care and to nobody else in the practice.
  • Notes from the talking therapies are held separately and are not shared without your agreement.
  • We will tell you what we hold, correct it if it is wrong, and explain who else has seen it.
  • We do not sell data, and we do not use your record for advertising.

For the full detail, including how long records are kept and how to ask for a copy, read our privacy notice, or write to privacy@larkmoor.example.

Close on a desk at Coppice Row, with notes being written up after an appointment.
