Who we are
Larkmoor Practice Group Ltd operates this website and the practice described on it. This notice explains how information about you would be handled.
This site is a design template running on fictional demonstration content. Replace this notice with one written for your own practice and jurisdiction before publishing.
What we collect
Contact details you give us when you book or write to us; clinical information recorded during your care; billing and insurance details where relevant; and basic technical information about how the site is used.
The booking flow and contact form in this template do not transmit or store anything. Nothing you type into them leaves your browser.
Why we hold it
To provide and record your care, to arrange appointments and tests, to bill correctly, and to meet the legal and professional obligations that apply to a healthcare provider.
We do not sell personal data, and we do not use your record for advertising.
Who can see it
The clinicians involved in your care, and the practice team members who need it to arrange and bill for that care.
Notes from the talking therapies are held separately and are not shared with the rest of the practice without your agreement.
Where an employer or insurer pays for your care, they are told that an appointment took place. They are not told what was discussed, and no report is shared without your explicit written consent.
How long we keep it
Clinical records are kept for the period required by the professional and legal standards that apply where the practice operates. Everything else is kept only as long as it is needed.
Your rights
You can ask for a copy of what we hold, ask us to correct it, ask who else has seen it, and object to particular uses of it.
Write to privacy@larkmoor.example and we will respond within the period required by law.
Cookies
This template sets no cookies, uses no analytics and loads no third-party scripts. Fonts are self-hosted at build time, so no request is made to a font provider when the site is viewed.
If you add analytics or an embedded service, update this section and add a consent mechanism appropriate to your jurisdiction.
